Can I Be A Patient?

New patients may schedule their first medical appointment on the same day as their eligibility screening.

 

Patients Must:

  • Live in Rockingham or Caswell County.

  • Have no health insurance.

  • Be at 250% or below of the federal poverty level for your household size.

 

Paperwork Needed:

  • One recent bill.

  • Last year’s IRS tax return (1040 with schedule C if filed, or 1040EZ.) If no tax return was filed, the Clinic has a form to fill out stating no return was filed.

  • Bring a list of all sources of income for you and all members of your household, including pay stubs.

  • Your photo ID.

  • Last month’s bank statement (checking & savings.)

To determine eligibility, one must provide a recent bill, last year’s IRS tax return (1040 with schedule C if filed, or 1040EZ), if a tax return has not been filed the clinic has a form to fill out stating no return was filed). Also bring all sources of income for you and your spouse including one month of current pay stubs, a photo id and the previous month’s bank statements (checking and savings).

Patients are re-screened for eligibility yearly based on their initial screening date.

Please bring all of your medications with you to all of your medical appointments.

 

 

Notify Clinic of Changes:

Please immediately notify our receptionist of changes in phone number, address, job or insurance status for you or any other qualified member of your household.

 

Free Clinic of Rockingham County Patient Expectations: 

  • Be cooperative in following the guidelines set by The Clinic.

  • Be courteous and appreciative of the volunteers and staff.

  • Be honest when talking with us.

  • Be kind and courteous to others in the waiting area and / or on clinic grounds.

 

Canceling Appointments:

Be considerate of the services The Clinic is providing.  Cancellations must be made at least 24 hours before the scheduled appointment.
This allows us to give your appointment to someone else who needs it.  Be responsible.  If you say you’re coming, COME.

All appointments are made on a first come first serve basis.

***Walk-ins are not accepted***

 

 

Closed or Full: 

If the clinic is closed or full, seek care from the health department, an urgent care facility, or the hospital emergency room (for emergencies only.)

 

Closings: 

The Clinic will be closed holidays, during bad weather, when there are not enough volunteers for staffing, and as otherwise necessary.  In cases of inclement weather, please visit our Facebook page for up-to-date information.
